Your Child's Genetic Report, Line by Line: What Each Word Really Means

The document you read ten times without understanding it


It almost always arrives the same way: an email, a phone call, or an envelope. Inside, three or four pages that seem written for another doctor. One line in particular keeps pulling your eyes back — the one that, in a handful of words and abbreviations, sums up everything science currently knows about what affects your child.


We've already explained, in earlier articles, what a "de novo" mutation is and how it happens. But no one has shown you how to read the line itself — the one that carries the verdict. That's what this article does: not the biology of the gene, but the grammar of the report.


We are not geneticists, and this guide never replaces a conversation with your medical team. It simply gives you enough to understand what you've already been told — and to ask better questions at your next appointment.


LMBRD2 Genetical report

🔬 One line, decoded

Reports vary from lab to lab, but almost all follow the same skeleton. Here's a typical line, built using the vocabulary that clinical genetics reports actually use:

Gene: LMBRD2 (NM_xxxxxx) — Variant: c.1448G>A, p.(Arg483His) — Zygosity: Heterozygous — Classification: Likely pathogenic — Origin: De novo


Let's take it piece by piece.

"NM_xxxxxx" — This is the reference number for the "transcript," the exact version of the gene's recipe the lab used to locate the variant. A single gene can have several slightly different reference versions; this number guarantees everyone is reading the same map.


"c.1448G>A" — The "c." means we're at the level of the coding DNA. At position 1448 of the sequence, the letter G has been replaced by an A. This is the exact typo, at the exact spot.


"p.(Arg483His)" — The "p." translates that typo into its effect on the protein, the final product built from the gene. At position 483 of the chain, the amino acid Arginine (Arg) has been replaced by Histidine (His). This is the concrete consequence of the typo on the finished product.


"Zygosity: Heterozygous" — You carry two copies of each gene (one from each parent). "Heterozygous" means only one of the two copies carries the variant, while the other is typical. For LMBRD2, this is the expected pattern: the associated condition follows an inheritance mode where a change in just one copy is enough (called "autosomal dominant").


"Origin: De novo" — This variant appeared for the first time in the child; it was not found in either parent when they were tested. This is the pattern seen in the vast majority of documented LMBRD2 cases to date.


📚 The 5 words that change everything: classification

This is the single most important line on the report — and often the most misunderstood. Genetics labs classify every variant on a standardized 5-tier scale (ACMG/AMP guidelines, used internationally):

🔴 Pathogenic

The link between this variant and the condition is solidly established. The lab is highly confident.

🟠 Likely pathogenic

Strong evidence points to a link with the condition, without reaching the highest level of certainty. In practice, this almost always guides care the same way a "pathogenic" result would.

⚪ Variant of Uncertain Significance (VUS)

Not yet known. Not enough evidence to say the variant causes the condition, and not enough to rule it out either.

🟢 Likely benign

The variant is probably a normal, harmless variation.

✅ Benign

Normal variation, documented as unrelated to the condition.

For a condition as rare as LMBRD2 — a single founding scientific publication to date (Malhotra, Ziegler et al., 2021) — many newly discovered variants first land in the middle category, before being reclassified over time as more families are identified worldwide.


⚠️ The VUS trap

Receiving a "variant of uncertain significance" is unsettling: it's neither a "yes" nor a "no." Two reflexes to avoid:

  • Don't read it as good news ("maybe it's nothing") or as bad news in disguise ("it must be serious and no one's saying so"). A VUS is neither — it's a gap in the data, not a hidden verdict.

  • Don't assume the label is permanent. Classifications evolve: a variant labeled VUS today can be reclassified as pathogenic — or benign — a year or two from now, as science advances. This is one reason participating in patient registries and research studies matters: every documented family helps move these classifications forward, for you and for others.


💬 Three questions to ask your geneticist

The report never tells the whole story. At your next appointment, these three questions usually fill in what matters most:

  1. "Has this variant already been reported in other people with LMBRD2?" — Places you in context relative to published or known cases.

  2. "If it's classified as VUS, what would it take to change that classification?" — Gives you a concrete sense of what's being watched for (a similar case elsewhere, a lab functional test, etc.).

  3. "Does this result change anything about my child's care or follow-up right now?" — Brings the conversation back to what matters day to day.

A report line can be read, decoded, understood. It says nothing about your child's smile, their progress, what makes them laugh. Understanding the report's vocabulary isn't an academic exercise — it's a way to take back a little ground from uncertainty, and to walk into the next appointment with sharper questions, not more fear.
